I recognize that the organization to which this application is being submitted is relying on the information contained herein. Accordingly, I attest and affirm that all of the information that I have provided is absolutely true and correct. I authorize the organization to contact any person or entity listed in this application, and I further authorize any such person or entity to provide the organization with information, opinions, and impressions relating to my background or qualifications. I voluntarily release the organization and any such person or entity from liability involving the communication or information relating to my background or qualifications. I further authorize the organization to conduct a criminal background investigation if such a check is deemed necessary. I agree to abide by all policies and procedures of the organization, and to protect the health and safety of the children/youth at all times.

I will consider my assignment a call from God to serve in church camping ministry. I accept this call knowing that camping involves hard work, long hours, with unforeseen challenges and rewards. I understand that I will be living and working with others, and commit myself to the principles of community, including respect for others, accountability, and direct communication. I will seek to nurture my Christian faith, so that Christ can be reflected through me to others. I understand that at any time if my conduct and/or attitude are harmful to the programs, facilities, or environment of Camp, either in the performance of duties, or due to inappropriate influence on other staff and campers, that my services will be terminated. Furthermore, use of alcohol, tobacco, or illegal substances during the work week is subject to immediate dismissal.
